Investigating Rotator issue

What happened

Team Lead called PCL over to the A Side Finish rotator on the BBF Line. As the PCL approached the area there were two Team Leads in the area already. When the PCL got to the area he asked if the Rotator wouldn 't run. One Team Lead was to the left of the PCL and one to the right. Before the Team Lead could answer the bucket fell out of the Rotator. The Team Lead to the left of the PCL had put some weight onto the bucket then the bucket fell out. The Team Lead to the left of the PCL was clear of bucket when it fell. The PCL managed to their feet out of the way in time as the bucket was falling. The Team Lead to the right however had both feet struck by the falling bucket. The bucket struck the right foot first then the left. The bucket struck the Team Lead 's right foot between the ankle and toes. The left foot was struck on the steel toe.
