EE was getting a stuck pallet from Level 2 he wasn’t paying attention to where his left hand was placed and had placed them on the guide rails where the wheels roll. As the pallet got pushed forward the wheels of the rack from which the pallet was stuck on began to roll forward crushing his left index finger.
