What happened
On [REDACTED] at approximately [REDACTED] a Wayfair supervisor working in bulk reported an Injury due to making contact with order picker forks with their lower legs. ;The supervisor Description of the Incident; The supervisor states that while searching an order picker for an associate badge they turned to walk and did not notice the PIV forks protruding into their walk path causing them to trip on the forks receiving a laceration on their left shin and pain in their wrists from catching the fall with a pain scale of 10 out of 10.;;The Initial Investigation Findings and Details; During the investigation the site found that the injury occurred while the supervisor was attempting to locate a badge that an associate had left on their order picker the [REDACTED]. The supervisor turned to mount the maintenance bike that they were using for transport and tripped on the PIV forks. Upon notification of the injury the supervisor accepted first aid.
