He was working with another employee replacing a coil of material.

The coil insert was not at the proper height or position on the transporting cart to transfer the new coil insert onto the machine. The other employee couldn 't get the coil insert on so he alerted the senior employee. The senior employee pulled the cart back and pushed the cart carrying the metal insert coils against the machine with excessive momentum resulting in one of the metal coil inserts to fall off the cart and hit the employee in the left should and left foot.

The employee suffered a broken foot.

The metal coil insert fell from the cart.

Contusion left shoulder and fractured left foot.
