Before the incident
Employee was completing setup of a molding job on machine [REDACTED] and was adjusting the orientation of the tool while inside the machine.
What happened
During removal of the remaining upper left-hand clamp the tool became loose from the platen and rotated clockwise resulting in the setup technician’s left middle fingertip getting caught between the lower left corner edge of the tool and the bolt securing the upper lefthand clamp to the platen.
