What happened
On [REDACTED] at [REDACTED]. Am employee was injured while pulling the cavity out of a mold in [REDACTED] Shop. The employee was performing a mold configuration on a [REDACTED] Shop table. While attempting to remove the cavities from the mold the employees ' hand was in between the cavity and the table. The cavity fell and lacerated their left middle finger. The employee was sent with an authorization form to Ochsner Urgent Care in [REDACTED]. The employee received 9 stitches and will again follow-up with the doctor 10 20 [REDACTED].
