Before the incident

EE came back to workstation and picked up a vent and it slipped and hit his right leg. EE told his lead he was not feeling good and asked the Lead to cover for him. EE did not inform Lead he was hurt or he was going home. EE says he noticed bleeding in his car and went to ER on his own and came back to the plant to drop off the paperwork [REDACTED]. EE was asked to come to the plant [REDACTED] to report the injury to call Medcor. EE did not come to plant [REDACTED]. EE came to the plant [REDACTED] around [REDACTED] to report and to call Medcor. He was advised self-care. EE did not like the recommendation. EE called Medcor again and has an appointment [REDACTED] at [REDACTED] with facility in [REDACTED].
