Employee was looking for hazardous conditions in the yard after having walked through facility reviewing PPE signage. She checked the propane storage lockers and was headed over to check the chemical station next to the water treatment unit.

She was headed to the chemical storage area and walked next to some materials left out: gratescovers of water treatment unit on a pallet. Her right pant leg got caught on the corner of one of the grates and she fell hard on the concrete. She fell on both knees and held her left hand out to break the fall and hit her index finger and thumb.
