Trying to remove jammed plastic from the grinder. Employee removed access panel and did not lock out the machine.

Employee removed the access panel off the back of the grinder to try to unjam it without performing lock out tag out procedures and proceeded to put his arms in the machine. A co-worker passed by and did not see the employee working on the machine and turned on the machine.
