The employee came to me and stated that his foot was swollen and that he could not finish his shift and that he wanted medical attention. When asked what caused the injury the employee stated that he had hurt his foot while stepping over a conveyor on [REDACTED] originally. He then stated that he reinjured the foot on [REDACTED] while cleaning the brew room and that it had been swollen ever since. The employee stated that he did not report the initial injury on [REDACTED] until the following day but was unsure who he made the report to.
