Before the incident
Employee was investigating a low pressure alarm on the tertiary filters at the WWTP removing caps from a pump line to make sure there was no ice in the line.
What happened
The employee went to restart the backwash pump and when he did water sprayed in the air due to a cap not being put back on the line. The employee went to replace the cap and when he did he slipped on ice that had formed on the grating from the water that sprayed from the backwash pump.

Employee struck his face on the grating causing a laceration above the eyebrow that had to be glued. He also chipped a tooth.
