What happened
Hotmelt spray associate went to empty her trash into the large trash bin at the value stream support area when she did this she pushed the trash down with her hands to make more room whereas it was almost full. These trash bins have a table top addition for our value stream support associates for making storing boxtops labeling boxes. As the associate was removing her hands from the trash she came in contact with a metal bracket that was in the center of the table
