Associate states that on [REDACTED] at around [REDACTED] he was pushing loads from the paint booth into the oven and he began to feel a jolt like sensation of pain down his lower back to his hips and into the front of his legs mainly on his quadricep muscles on both legs. Concentra clinic placed the associate on restricted work for the following 4 weeks and prescribed muscle relaxers for the symptoms.
