What happened
The employee took a brick over to the [REDACTED] Cutter to cut off a piece of non-conforming rubber. There was paper on the brick causing the non-conforming rubber. The employee had both hands on the lever of the [REDACTED] Cutter and the lever came off (out of position) and he braced his hand (left) on the table. The lever struck his left ring finger and it happened too fast for him to react. The [REDACTED] Cutter was missing a retaining ring which holds the lever in position. The employee did not know the retaining ring was missing. Further no one knew the retaining ring was missing and it was never reported to management. The [REDACTED] Cutter was immediately removed from the floor and sent to the Tool Shop in Building A for adjustment.
