Associate completed unloaded the door as instructed.

A load bar and pallet boards were used to hold a broken dock door open. Removed these props after unloading the all pallets from the trailer led directly to the associate having a concussion and a laceration on her scalp that required 3 staples to close. A wheel from the roll up door broke lose when removing the two items that held the door open.

Associate has a laceration on their forehead that required 3 staples to close. Associate also diagnosed with a concussion.
