EE was checking for proper rotation of a hand-held die grinder (dremel-style tool) w a rubber wheel attachment

Employee triggered the tool and the rubber wheel split broke separated from the center piece of the wheel and pieces flew in several directions. We believe a separated piece of the wheel hit the top edge of [REDACTED] 's safety glasses with significant force causing the top edge of those glasses to press into his eyebrow.
