What happened
Associate was not following process. She picked up two tubs of parts and attempted to place them on the conveyor belt. She did not lift them high enough to clear the conveyor which caused her to strike the conveyor with the tubs. She lost her balance and fell backwards into the cart she had just taken the parts from.
