What happened
Employee opened air valve to release pressure to open gap between two non-rotating rollers to feed paper through. As pressure was released it allowed roller to release pinching elbow between the frame and roller. We were unable to accommodate restrictions of no use of right arm due to the need to climb fixed ladders on the press and thus the extensive lost time for this injury.
