At around [REDACTED] the activity host came to this writer for help with a resident who was agitated during bingo in the activity room. Patient kept walking around unassisted and was unable to be redirected. Resident started walking down the hallway and refused to sit in wheelchair. This writer helped resident to ambulate to her room while walking resident became combative and tried to slap this writer in her face. This writer tried to block the slap from resident by attempting to hold her hand resident proceeded to tackle this writer and both fell. This writer fell on her right side affecting right knee shoulder arm right hand and wrist and right lower back. Small scrape to right knee and along three digits of the right hand. Body aches and pain 510 during the remainder of this shift.
