What happened
The Pad Eye was hooked with a strap that was connected with the 20-ton crane. Technician [REDACTED] stated that he was maneuvering the crane before grinding the pad eye off and in the process of moving the crane he stated there was a delay on the remote which caused the crane to tighten up the slack. The force and pressure caused the pad eye to come off striking the employees right hand.
