What happened
Three employees were tasked with joining the shells of the heat exchanger using z-dogs. To adjust the bolt on the z-dog the employees used a wrench with a 4.5’ steel pipe to create leverage. As the employee was pulling the pipe towards themself the employee’s left thumb hit the adjacent z-dog which resulted in the loss of the employee’s thumb nail.
