Operating Elevator

[REDACTED] and [REDACTED] were transporting six 6 pieces of [REDACTED] 8 drywall sheets on a dolly cart from one level to another using the staff elevator. While exiting the elevator on Level 7 the front wheel of the dolly became stuck on a loose piece of plywood that was laying on the floor just outside the elevator. As a result of the obstruction the dolly tipped over causing the drywall sheets to shift and strike [REDACTED] on her left arm and leg. At the time of the incident [REDACTED] was positioned directly in front of the elevator assisting with guiding the load. The impact occurred as the load lost balance due to the uneven surface created by the plywood obstruction.Employee went to clinic without notifying [REDACTED] and RX medication for inflammation.
