What happened
[REDACTED] strained his right arm while cranking the 7 head tailstock closed. Even after spraying the ways with lubricant the mechanism remained hard to crank. [REDACTED] felt and heard a pop near his elbow. He reported the incident applied ice and later went to the emergency room. Xrays showed no fracture but OIO diagnosed a bicep tear that required an MRI. [REDACTED] stated the injury could have been avoided if he had stopped when the tailstock first bound up. The root cause was mechanical malfunction of the tailstock. Corrective actions include repairing the 7 head creating a ratchet adapter exploring a pneumatic solution sharing the incident with associates and training leaders on proper injury response. [REDACTED] returned to work on [REDACTED] with restrictions after 82 lost time days.
