Operator did not have his edge guide set correct when threading up a coil and when it went into the head he did not have...

[REDACTED] was threading material into the slitter line when the operator side edge guide was not set correctly. This left no side trim requiring him to back material into the shear. When the scrap was cut the wide trim became stuck in the chute. [REDACTED] opened the gate and tried to pull it by hand. The scrap fell and pinched his left index finger against the platform resulting in a cut that required five stitches. He used his hands instead of a no touch tool. Maintenance welded a plate on the drive side to prevent scrap from catching again. The root cause was scrap getting stuck and being pulled by hand. Corrective actions include sharing the incident welding plates to stop sticking reinforcing the use of no touch tools retraining on proper threading and checking other Minster slitters for similar conditions.
