EE stated that he felt soreness in his right shoulder on [REDACTED]. He did not report it. On [REDACTED] after lifting 50 pound bags he felt a sharp pain in his right shoulder. He did not report it. On [REDACTED] he went to the Freeport ER for examination. Excused for work for [REDACTED]. Dr. Suspected a rotator cuff injury. On [REDACTED] EE sent the ER Doctor 's note. EHS Director interviewed EE on [REDACTED]. Sent EE to PIC for follow-up. PIC wrote Rx and restrictions. EE took himself off on [REDACTED] and took PTO. EE returned to work on [REDACTED].
