Teammate was operating a Landis 1.5inch cut threader. He reported he had processed 10-12 threaded rods that day prior to the incident

Teammate reported he performed the process as described below to produce the 10-12 threaded rods prior to the incident. i. Teammate did not push the Stop button to ensure the threader is turned off and the threader head is not turning ii. Load the rod to be threaded into vice iii. Tighten rod in vice using both hands on tightening wheel to ensure the part will not turn as threads are cut iv. teammate did not turn on the threader on it was on during loading v. Use wheel to roll the carriage holding the rod into the threader vi. Once threading is complete the threader will automatically disengage unclamp the threaded rod vii. teammate did not turn threader off when threading was complete turn the threader off viii. teammate did not follow this step after the threader has stopped turning loosen the vice ix. Remove the rod from the threader and repeat the process c. Mr. [REDACTED] loosened the vice that prevented the rod from turning before threading was complete causing the rod to turn d.
