Before the incident
EE was walking outside of the chair 2 patrol shack He stepped in a hole and hyper extended his knee At the time he thought nothing of it and continued to work with no lost time or modifications for 3 weeks [REDACTED] he notified me that we was not able to perform his job duties and would like to be seen at the clinic I made arrangements for him to be seen the following day The clinic told him no skiing but he could work modified duty He has decided he does not wish to work with modified duty and will sign the declination for today He has a follow up occ health appointment on [REDACTED]
