What happened

[REDACTED] says that he thought he tripped on a piece of wood in front of the ice machine. He also noted that the floor was wet due to the drain grate being in front of the ice machine. He fell on his front. He used his hands and arms to try and catch himself. The box fell on the floor. He said he tripped a second time- but he could not remember why he tripped. Due to [REDACTED] tripping in the back and where it happened- it did not happen on the cameras. [REDACTED] said he is clumsy and that he did trip at home that same day.
