Teammate was stacking bolts on a pallet. He was moving bolts from one pallet to another. He noticed one of the bolts on the destination pallet was not placed correctly. he attempted to adjust the bolt 's position on the pallet when the bolt rolled off the pallet from a height of approximately 3 feet onto his left foot striking his foot behind his steel toe.
