Per [REDACTED] Camacho at around 20:30 on 05 04 2025 he was dropping off order S263974 to dock door #74. He saw that there was no space on that dock door and decided to drop off the order further on the dock. This is when he stated he noticed that the pallet was stuck on his forks of his Center Controlled Rider Jack # 7735. He went further down the dock in front of Door 78 by the offramp entrance where he saw a pole that he believed that he could unstuck the pallet. As he was trying to get the pallet of his forks he turned towards the backrest of the jack and tried to maneuver the jack frontwards. When this happened he stated that everything happened fast and does not know if the machine abruptly stopped or went forward in that instant. This is when he hit his mouth on the metal rail of the backrest of the jack. He split open his lip and stated he felt the pain on his front teeth as he smacked them also.
