What happened
Smashed ring and middle finger in hinge welder on Salvo. Running Salvo hinge welder in manual mode and employee placed hinge in by hand and activated the weld cylinder switch with his right hand while not clearing his left hand before it triggered. This caused the two fingers to get smashed between the weld tips. Taken to the nurse station to look at his fingers and given ibuprofen and a cold pack. [REDACTED] contacted [REDACTED] and had Maintenance lock out tag out the hinge welder on the Salvo.
