What happened
Employee was changing the guide rail set up on the garage door machine and had to go under the table to loosen the nuts and bolts that hold the rails in place. While he was under the table loosening the nuts and bolts he cut the side of his left leg below the knee on a piece of broken glass that was on the floor that he did not see. There was a glass bottle of tea or juice that was dropped at some point in the past. When the glass was cleaned up this piece of glass was missed and became stuck to the floor under the table.
