What happened
Patient was asking to go on the bedside commode and wash her hands. As we were going back to patients ' bed she started punching me and trying to reach for my face. Once I managed to get her back in bed she kicked me in the stomach and chest while I was trying to call for help from other staff. Patient was also hitting Bitting and being verbally abusive to other staff. We got her back in bed again and she launched out of it causing her to lose her balance hit her face on the wall and fall.
