Before the incident
While starting a brush clean of the Line 1 Vibratory Tube EE passed out and fell off a 3 step ladder approximately 3 feet While looking into the incident it was noted that the operator did not do an O2 check after opening the system and before beginning work of the brush clean EE began work without having the O2 monitor on his person EE was brushing the excess powder out of the tube when he passed out EE was caught by another employee so no major injuries were sustained It is likely that the Nitrogen in the system did not have enough time to vent before EE began work
