Building operation and maintenance

Description: While setting up a fall arrest tripod the employee tripped on a valve box resulting in a right knee and ankle injury. Suspected Cause: Employee stepped down and the edge of the sole of his boot caught the edge of a valve box. The majority of his boot did not have support and his ankle rolled and he fell to the ground. Immediate Actions Taken: Another employee was approximately 30 feet away inside the pump station building. He heard the tripod crash and went to the employee immediately. He then contacted his superviso and EHS manager who directed to transport employee to the emergency room at St. Luke 's Hospital.
