Description: [REDACTED] at around [REDACTED] the Assistant Project Manager at Discovery Bay was working with a maintenance technician on an underground electrical vault at [REDACTED] on the Digest Pond Aerator. The APM was reaching into the pit vault to troubleshoot the wiring as the heavy lid fell striking the top of his forearms just above the wrist. The impact caused a small laceration on his right forearm. Findings from the investigation: The work was outdoors and it is believed that a gust of wind blew the lid shut. It is slightly windy but not above normal for our area or throughout the wastewater plant. The lid is heavy and old but it does have working lid locks. The lid was tested several times and with body weight and we were unable to disengage the lid locks so wind couldn 't have been the cause. The lid locks are automatic as long as the lid is totally open. APM was wearing PPE and also had long sleeves and a jacket which added some protection to his arms from the falling li
