Was assisting pt. from the bathroom back to bed. Pt. was becoming slower and leaning on both staff more. Encouraged pt. to continue to bed (was between bathroom and bed). Pt. started to fall writer and [REDACTED] caught and lifted onto the side of bed. When pt. started to fall the weight and turning of catching him and then lifting into bed strained back and neck.
