Helping breakdown an AGW

What happened

While suspended in the horizontal position and before placing the AGW on the cribbing one of the guide wheel clamps came off the inner AGW frame causing the inner frame to sag and prevent setting the AGW on the cribbing. Our [REDACTED] attempted to lift the inter AGW frame and set the clamp back into place but he was struggling to lift the frame by himself. The Employee came over from outside the tank to help hold the frame and try to help our [REDACTED] clamp the frame into place. The [REDACTED] and The Employee were working in a kneeling position. While trying to set the clamp. The Employee then fell from a kneeling position to their stomach on the tank floor. The [REDACTED] noticed that The Employee had shallow breaths and wasnt moving. Neither our supervisors onsite first responders nor the paramedics were able to revive The Employee. Casue still under investigation and TBD
