Employee was helping tarp 84 x 8 ' dry cast concrete pipe.

While tarping the concrete pipe by using an overhead crane the tarp got hung up on a pipe. The employee grabbed a 6 ' rolling platform ladder with hand rails to climb up and free the tarp from the pipe. The employee was tugging on the tarp. When the tarp became unstuck from the pipe the employee lost his balance and fell off the ladder platform landing on the ground.
