Before the incident
The employee was jackhammering concrete out of a precast box culvert. The concrete was getting jackhammered so that the box culvert could be converted into a left hand bend. The employee was using a battery operated jackhammer drill.
What happened
While on the inside of the structure the employee was jackhammering while standing on the second step of a 4 ' step ladder. The jackhammer bit got wedged between some concrete and the culvert reinforcement. The employee yanked the stuck bit from the concrete casuing him to lose his balance and fall to the ground.
