On [REDACTED] at approximately [REDACTED] the forklift operator was driving forward without a load while supplying trays to Production just north of the [REDACTED] palletizer in the northeast section of the [REDACTED]. As they were driving they reported that the support beam was in their blind spot behind the left mast and they struck the concrete base with the tip of their left fork. They reported neck pain at the middle of the base of their neck as well as pain to their left rear and side deltoid as a result of the sudden stop. Based on MBI treatment they are leaving work for the remainder of the shift.
