At around [REDACTED] as our uniform supplier was clearing dirty clothes and hangers from the locker rooms and heading back to their truck a hanger fell from a dirty clothes cart and remained on the floor. Several associates (11 in total) walked past the hanger without an incident or noticing it. The associate involved had arrived early to the facility for a training and made two trips to the restroom passing over the hanger each time. On her third trip she stepped on the hanger slipped forward and fell to the floor landing on both palms and knees.
