What happened
An Equipment Operator (EO) was driving a [REDACTED] northbound in the main drive aisle toward the middle command center of [REDACTED]. As the EO approached the [REDACTED] area an Automated Guided Vehicle (AGV) unexpectedly exited an aisle at speed traveling east to west into the main drive aisle. The EO did not see the AGV until it entered the aisle. The operator took sudden evasive action to avoid a collision. The [REDACTED] became unstable during the maneuver and the EO fell to the floor.
