Before the incident
TM was removing dough rom the chunker.
What happened
TM was removing dough by hand due to an equipment issue shut down. Once the level was down far enough the TM attached the handle to the takeaway conveyor and tried to remove the takeaway conveyor from under the chunker. The TM pulled as it is heavy and it was stuck due to the dough. When it moved and broke free the handle detached and the TM fell backwards and bumped their head on the Vemag.
