Before the incident

What happened
The material on the stacking table was building up and became jammed. Parts began to get thrown off the stacking table from the mill running while the parts were jammed. The first set of two that came off missed the employee caught the 2nd set that came off while trying to remove material employee tossed the ones he caught down and while he continued trying to pull material off the stacking table the 3rd set that got jammed and sprung off the table struck employee in the face.
