Before the incident
The associate was unjamming a hoist.
What happened
A container of linkage components which is lifted by a hoist became jammed near the shoot within the guarded enclosure. The associate opened the interlocked guard and began unjamming the container while with a crowbar. There was slack in the chain between the hoist and jammed container. Therefore when the associate unjammed the container it fell onto them and struck their head.

The associate sustained a laceration to the forehead (8 sutures) laceration to the L cheek and a laceration to the nose.
