The associate was loading and unloading gears to Lubrite as well as moving dunnage.

The associate leaned against dunnage which was propped against a pedestal floor fan. The fan began to fall and the associate attempted to catch it. As the associate attempted to catch the fan the fan twisted and the shroud bent allowing the associate 's face to make contact with the blades of the fan.

The associate sustained lacerations to the R cheek and eyebrow requiring several stitches.

The fan 's blades directly harmed the associate.
