associate was cutting open a box while picking in zone 5A

while cutting the tape on a cardboard box the tape on the box bundled and caused the Klever cutter to get stuck. The associate used force to cut through the tape and did so with his lieft hand in the way. After further review the Klever cutter was determined to be dull and associate did not have PPE.
