Pushing a stub roll across the floor
What happened
Concussion Restriction: The team member was pushing a stub roll about 5 inches wide of slip easy paper across the floor with the roll flat on the floor. The roll started to unravel and the team member stepped on this paper. This caused the team member to slip and fall with his head striking the floor. The team member felt fine and worked the remainder of the shift. When he went home he began to get a headache and when he woke up the [REDACTED] he felt dizzy and nauseous. He worked a portion of the [REDACTED] and was evaluated by the nurse. The [REDACTED] he came in and was evaluated by the doctor. The doctor recommended no work around moving equipment or operating PIT 's.
