Opening a door on the pulper

On [REDACTED] at [REDACTED] EE was at stock prep 1 central refining pulper. EE was in the process of opening the access door to put in bales of wet lap when they drove forklift up to the door to push it open. EE 's foot slipped off of the clutch and the forklift nudged forward. EE 's left ring finger and left middle finger got wedged between the roll cage pillar and the door. EE had a level 10 pain with bleeding and bruising. EE 's fingernails on both fingers appeared to be falling off. Security ran water on the fingers until the bleeding subsided applied hydrogen peroxide gauze and wrap. EE was transported to hospital for further evaluation. EE 's supervisor was notified.
